The formula version record
“D-76” is the name of a formula. “D76-EB-003” is the name of a litre. FV-1 and FV-2 are what that second name points at, and they are two sides of one sheet: print them back to back and file one per batch you mix. A result that cannot be tied to a specific mix is not evidence about a formula, which is why every strip, plot and conclusion in Part IX cites a code rather than a recipe.
FV-3 and FV-4 are the variant record, one per proposal: the parent, the single change as a quantity, the evidence it rests on, the prediction stated so that it can fail, the test that decides it — and then what actually happened. A proposal with two changes in it is a proposal that cannot be evaluated, which is why there is room for one.

Every field on these sheets is one a written page of the course asks you to record. Those pages are linked here, and the build fails if one of them stops existing.

What these sheets do not carry
- The printed version code is a student identifier. The browser notebook offers a separate catalogue-parent field; neither record certifies that a variant is safe or equivalent to its parent.
